Our library does not keep incomplete applications. If any required document or payment is missing, you will need to make another appointment. You are expected to bring the following to your appointment:
Payments:
Two separate payments are required:
- Passport application (U.S. Department of State) fee: MUST BE PAID by CHECK (personal, certified, cashier's, traveler's) or money order. Payable to U.S. Department of State. Bring separate checks for each applicant. See the Department of State's current fees. U.S. Department of State does not accept cash or cards.
- Example: A family of four are applying. You will need four checks for the State Department. Please consider bringing an extra check.
- Example: A family of four are applying. $35 (service) + $15 (photo) = $50 per person. Total = $200 payable to Avon Grove Library.
Documents:
- A completed form DS-11 for each applicant. Forms must be printed one-sided and in the original format - do not shrink. Write legibly using BLACK INK ONLY. Do not use white out. If you make a mistake, print a new form. Forms are available at the library.
- Additional forms may be required
- Proof of identity: Government-issued photo identification document. (Must be the physical ID - not photo or digital copies) For minors, both Parents’ IDs are required.
- Proof of U.S. Citizenship: U.S. birth certificate (Original or Certified Copy; must meet requirements) OR Naturalization Certificate OR Fully-valid, undamaged U.S. passport (can be expired). Your existing U.S. Passport book and/or passport card will be submitted with the application.
- Passport photograph (photo service available at the library for $15). If you are bringing your own passport photo, make sure it meets requirements of the U.S. Department of State.
Children under 16 (minors) applying for a passport:
- Both parents must appear at the appointment with the child. If one parent cannot be present at the appointment, they must complete a notarized Form DS-3053 "Statement of Consent" and bring a photocopy of a government-issued photo ID (e.g., front and back of a valid driver’s license) of the non-applying parent. The attending parent must bring their government-issued photo ID in addition to the photocopy of the non-attending parent's government issued ID.
- If one parent has sole legal custody, legal documentation such as Court Order must be submitted.
- If one parent is deceased, a valid, original death certificate must be submitted.
- A certified birth certificate is required for children in order to prove parental relationship. If they already have a passport, a photocopy may be acceptable.

- At least one parent/legal guardian must accompany applicants aged 16 and 17 to show parental awareness.
- If the applicant has a learner’s permit, a junior driver’s license or valid school photo ID, they must bring physical documents as proof of identity. The existing U.S. passport may be used as either proof of identity or citizenship but not both.
